Welcome to Functional Therapy

Functional Therapy and Rehabilitation is a physical therapist owned out-patient practice founded in 2001 by Barry and Phyllis Levine. FT&R is a licensed Physical Therapy facility treating a wide spectrum of orthopedic, musculoskeletal, as well as neurological disorders and dysfunctions. We bring not only a great deal of experience to individual clinical treatment, but have established an efficient and effective "therapeutic environment," as well.

Services

At FT&R we treat both conservative (non-surgical), as well as post-operative diagnoses. Beyond the treatment of dysfunction associated with specific tissues and joints (foot, ankle, knee, hip, spine, shoulder, elbow, hand, wrist), we are experienced in the treatment of more systemic conditions, such as osteoarthritis, rheumatoid arthritis, and osteoporosis, for example. We also treat work-related and athletic injuries, having treated many youth athletes, the "weekend warrior," and on to the elite and professional athlete. Our therapists are experienced in treating pulmonary conditions and geriatric populations with generalized deconditioning and balance deficits.

Additionally, our therapists perform functional capacity evaluations (FCE), utilizing the MediGraph software; thus, allowing the results to be available within 24 hours of testing. For additional information please feel free to contact us at either of our offices.

About Our Staff

FT&R's clinical staff is comprised of experienced and licensed Physical Therapists only. We do not employ Physical Therapy Assistants or Aides. Each patient is seen on a one-on-one basis, by a licensed PT who will design a program specifically for each individual's needs and goals. We feel that this provides the best opportunity to achieve the clinical outcomes and results we are seeking and which our patients deserve.

Experience is not an issue at FT&R. Our staff jointly has over 50 years of experience in the delivery of Physical Therapy. We bring not only a great deal of dedication and experience to individual clinical treatment, but have established an efficient and effective "therapeutic environment," as well. It is something you need to experience to appreciate.

Insurance

FT&R is a certified Medicare provider. We accept most private insurance, managed-care contracts, and Workman's Compensation. However, due to the frequently changing managed care system, please call to confirm your coverage and benefits prior to the first visit. Our office staff will be pleased to assist you and verify your coverage.

We are now accepting most major credit cards as a payment option.

Patient Information

Initial Appointment Planning

During your initial appointment, your physical therapist will conduct a comprehensive evaluation. This will consist of collecting a medical history and obtaining relevant and necessary tests and measures based on each individual's presentation.

What to bring on your first day:

1. A prescription or referral from your physician
2. Insurance Card
3. Driver's License
4. A list of current medications
5. Comfortable clothing and shoes appropriate for exercise.

*Please arrive 10-15 minutes earlier than your initial appointment in order to allow ample time for all paperwork to be completed*

The new patient forms are available to complete on-line or to print and complete prior to your first visit. If you prefer to email the completed documents before your first visit, you may attach the forms in an email with "patient forms" in the subject box and send to ftr@functionaltherapy.net
