The GoodLife and FTR family is an independent PT owned business dedicated to our patient’s and profession. We provide the highest quality patient care based on the most current research and methodology. Our therapists participate in ongoing certification programs to help our patients on their journey to better. We believe your rehabilitation deserves our full attention which is why we take the time to perform routine re-assessments and provide patients with educational materials to further progress their recovery.

Meet The Therapists

Jim Donahue

Jim Donahue

PT, Doctor of Physical Therapy

Jim has been practicing physical therapy for nearly 18 years. He received his Masters of Physical Therapy from Governors State University in 1999. He later attended Rosalind Franklin University earning his Doctorate of Physical Therapy in 2006.

His primary area of interest is orthopedics; however, he completed his Doctorate training studying the current concepts for the treatment of patients with rheumatic diseases. He has begun furthering his understanding of vestibular problems and the dysfunctions that arise from balance difficulties.

Jim enjoys spending time with his family. He attends regular TRX and modified CrossFit training classes with his wife. He spends his summers as a spectator watching his son play baseball and his daughter delight in her love for dance. He has developed and been involved in several off-season training programs for local travel baseball teams, and has lectured on overuse injuries in the overhead athlete. He has attended countless courses on shoulder and elbow injuries. He also finds time to attend and review articles pertaining to Chicago’s Dance Medicine Journal Club.

Jim has served as a member of the Advisory Board for the Masters of Management Health Care Administration at Robert Morris University Graduate School of Management and has been a member of the American Physical Therapy Association since 1997


Dana Weith

Dana Weith

PT, Doctor of Physical Therapy

Dana has been practicing Physical Therapy with Functional Therapy and Rehabilitation since 2009. She graduated from Bowling Green State University in 2006 with a bachelor’s degree in Biology where she was also a member of the university’s swim team. She further continued her education at Midwestern University in Downers Grove where she obtained her Doctorate in Physical Therapy in 2009.

Dana currently lives in Chicago but grew up in the Tinley Park area. She loves spending time with her husband, son Brayden and her Siberian Husky Dakota. She is passionate about travelling and leading an active lifestyle in which she participates in regular crossfit, running, and triathlons. In the winter months she enjoys venturing out to the ski slopes and during the remainder of the seasons she likes hiking with her family.

Dana is also a member of the American Physical Therapy Association (APTA), a credentialed clinical instructor, and is pursuing her certification in MDT, FMS, and vestibular rehabilitation.


Kate Ehmann

Kate Ehmann

PT, Doctor of Physical Therapy

Kate graduated from Northern Illinois University in 2012 with a Bachelor of Science in Health and Human Services, a minor in Communicative Disorders, and a minor in Nutrition Dietetics and Hospitality. She further continued her education at Governors State University (GSU) where she received her Doctorate in Physical Therapy in 2015.

While attending GSU she was an active member of the Physical Therapy Student Association and played for the GSU volleyball team. Kate is a member of the American Physical Therapy Association (APTA).

She is currently pursuing a certification in Vestibular Rehabilitation. in her spare time, she enjoys attending Pure Barre and SPENGA classes, hanging out with her friends and experimenting with family recipes.


Mickey Shah

Dr. Mickey Shah, PT, D.Sc. OCS, FAAOMPT, Cert.MDT; Certified Vestibular Rehabilitation Specialist.

It was his strong desire to help others and a simple treatment philosophy of treating patients like family that motivated Dr. Shah to open his practice over 10 years ago. As a university professor and clinical researcher, Dr. Shah brings innovative and evidence-based practices to examining and treating his patients. Dr. Shah also maintains a specialization in Vestibular Rehabilitation, including Concussion Management, as well as a specialization in Spinal Disorders including: Headaches, Neck Pain, Low back Pain and Temporomandibular Joint Dysfunction.

Dr. Shah lives in Frankfort with his wife and two daughters and enjoys putting his ‘handyman’ skills to work on small projects around the house. However, he is quickly realizing that small projects unexpectedly turn into big projects. And when off the clock, he enjoys spending time outdoors hiking, playing hockey and skiing.

Steven Tijerina

Steven Tijerina

PT, Doctor of Physical Therapy

Steven Tijerina is a Physical Therapist at Goodlife Physical Therapy in Orland Park, IL. He was born and raised in the south suburbs of IL. He has a passion working with patients suffering from orthopedic, neurologic, and vestibular impairments. Dr. Tijerina believes that every patient should receive the highest level of care possible, and believes that every patient should be treated like close family with personalized one-on-one care.

Steven graduated from Governors State University with a Doctorate in Physical Therapy. After graduating from school, Dr. Tijerina has pursued specialized training for rehabilitation of patients with Parkinson’s Disease and vestibular disorders. He completed a competency based program for vestibular rehabilitation endorsed by the American Physical Therapy Association and is certified to use the LSVT-BIG program, a method designed to help treat patients with Parkinson’s Disease. Currently, Dr. Tijerina is pursing certification in the MDT approach (McKenzie Method) to treat spinal pain and dysfunction. Dr. Tijerina also enjoys helping patients with chronic foot pain, jaw pain and knee pain.

Apart from treating patients, Steven loves to spend his free time outdoor hiking and serving his community. Dr. Tijerina enjoys hosting patient support groups and community workshops to help educate the South Suburban Community. Dr. Tijerina also uses his free time to help educate other physical therapists on Vestibular rehab, acting as a Lab instructor and Course Liaison for a Physical Therapist Education Company.


Dale Schuit

Dale Schuit


Dale has been a practicing physical therapist for 41 years. He received a BS degree in Physical Therapy from St. Louis University in 1976. In the following years, he has received a Master of Science degree in Physical Therapy Education from Ohio State University (1979), and a PhD in Kinesiology from the University of Illinois at Urbana-Champaign (1988). He has held full time academic PT faculty appointments at local universities; he currently is an Associate Professor in the Department of Physical Therapy at Governors State University.

Dale is an active member of the American Physical Therapy Association (APTA), and has attended and provided professional presentations at national and international physical therapy conferences. He has produced a number of professional research publications.

Dale is an avid golfer and a certified Titleist Performance Instructor (TPI) who performs Golf Screens at our Homer Glen location. He is a proud grandfather who enjoys spending time with his family, hiking, and photography.

Phyllis Levine

Phyllis Levine

PT, Doctor of Physical Therapy, Emeritus

Phyllis founded Functional Therapy and Rehabilitation, with her late husband, Barry, in 2001. After 47 years of practicing, she has recently started the next phase of her life. She is no longer seeing patients at FTR but is still an integral part of our team as a consultant. She currently spends her time participating in pro bono physical therapy, teaching, painting and traveling.

She continues to serve her community as an elected trustee on the Board of Trustees of the local library and is a clinical consultant to the Advisory Board of Governors State University. She is the mother of 3 grown children and actively enjoys many of the cultural aspects of life in Chicago.

Phyllis graduated with a BS in PT in 1970 from St. Louis University. She returned to Rosalind Franklin University to complete her DPT in 2007. During her career, she was a visiting instructor in the PT programs at Governors State University, Northwestern University, and Rosalind Franklin University. She has presented many topics at state and national meetings and published refereed articles and book chapters. Most of her publications and presentations are on the topic of scoliosis, spine problems and rehabilitation, gait analysis, sport rehab and pulmonary medicine.

Robin Clark

Robin Clark

Office Manager

Robin is one of the friendly voices and faces that our patients hear and see when coming into our physical therapy clinics. She has been with the company for a little over 3 years and is excited to grow with the company. As the office manager, she travels between our 3 clinics and handles billing, accounting, and patient records. In her spare time, she enjoys spending time with her family and friends and loves to bake with her niece and nephew. She is very passionate about ministry work and loves her cats.


Laura Petritis

Laura Petritis

Front Desk Coordinator

Laura is the Front Desk Coordinator at our Orland Park location. She handles all front desk operations including patient scheduling, patient registration, insurance verification, and patient authorization. She has a Bachelor of Science degree from Lewis University in Human Resources and Psychology, and loves working with people.

Laura is married and has two daughters. She loves going to concerts and going shopping with her girls. Laura and her family are also involved with Special Olympics Illinois.

Closing Statement
